It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. There is an option for cancelling an appointment on our telephone system. You will not have to wait in a call queue to do this.
You can also use SystmOne Online and NHSApp to cancel an appointment.
Stratton Primary Care Hub offers appointments for people with minor illness symptoms on some week days. You will see either an Advanced Nurse Practitioner or a GP for your appointment.
You can access the hub only through your usual practice who will advise if the hub is appropriate for you, using specific eligibility criteria. You will need to be able to get to Stratton Hospital to attend your appointment at the Hub.
Our Home Visiting Team can visit you at home if your condition means you are unable to attend at a Medical Centre.
Please ring before 10:30am to arrange a visit. You will be asked a number of questions about your condition on booking.
Visits are conducted throughout the day over a wide area so we cannot specify an exact time when you will be visited. If your condition deteriorates before we are able to get to you and you need urgent assistance please call 999.
Every patient at our practices is allocated a named GP. This is necessary for legal and administrative purposes. You can, however, see any of the GPs working at your practice, expressing a preference when you book. This does not have to be your named GP, although you may wish to see the same GP for continuity of care.
Information coming back after referrals to hospitals and other healthcare providers, will always be sent back to referring clinician.
We also offer early morning appointments from 7.40am at HolsworthyMedical Centre.
Telephone appointments are available with most of our heathcare professionals. If you are being booked into our Critical Care Team (On The Day Team), then we will not be able to give you a time.
If you are using a mobile phone, please keep your phone on and make sure you have a strong signal.
We have a texting service which allows you to receive confirmation and reminders about some (not all) appointments and to cancel them.
To have this service you will need to register by completing a consent form.
