Online access can be granted to our patients. There are different levels of access which can give you a range of options, including prescription ordering, booking appointments and seeing test results by phone or computer. To grant online access, we will need to have proof of identity, plus a completed form. Please use the links below to read our information leaflet before you apply and download the form that you need to complete.

Please print and complete the form and bring it to the surgery with two forms of ID, one of those being picture ID. 

If you are unable to print the form, please visit your surgery reception to pick up a copy.
