REGISTER AS A NEW PATIENT

The doctors welcome new patients who live within our practice areas.

From September 2024 we are no longer accepting new patients who are registered outside our practice area. 

All new patients complete a health history questionnaire and are offered a health check if they would like one.

Pre-registration With the Practice

Our practices are now part of the new GP Registration Scheme which allows patients to search for and register with a local GP without the need to visit the practice in person. 

If you wish to register, click on the links below to open the registration form. You will need to choose which medical centre you wish to register at: either Holsworthy and Hatherleigh or Stratton.  Please also complete the patient questionnaire form for the time being (so we can establish a few brief medical details in advance of the arrival of your medical records). It is helpful if we are able to see two types of ID after registering with us. You may bring them in with you for your first appointment.

Please note - until we have approved and added your registration details to our local system, you will not be registered with us. Please DO NOT fill out an accuRX online consultation until you are fully registered. Please contact us if you need to confirm your registration has been approved and processed. 

Registering for on-line access

Once you have been registered with a practice, you can apply for on-line access.

If you wish to register for on-line access to your patient record, order repeat medications, book appointments and check on test results etc. please click HERE

To enable us to verify your identity, you will need to provide ID to get on-line access to your medical record.

 

PROXY ACCESS

Proxy access allows someone other than the patient to access and manage parts of their GP online services account. The proxy is given their own online access first, rather than using the patient's login details. This may be used by parents or recognised carers of young children, and recognised carers of adults.

To set up proxy access you will need to contact the GP surgery of the person who wants help, to request access. You should do that in writing, request a form from reception or download the proxy access consent form (right) and hand this in at reception. You may both have to provide photo ID in order for us to verify your identity/identities.

The GP surgery will then make sure the person giving access understands and agrees to it, where appropriate. They will be asked to sign the consent form. This will then be added to that patient's record.

The GP surgery will then decide what access, if any, is appropriate, and make the changes in their clinical systems to set it up. As this process needs to be done carefully, please be prepared to wait for all the checks to have been made. We will inform all parties involved of the outcome of the request once a decision has been made.

Temporary Patient Registrations

If you are ill whilst away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

FIND YOUR NHS NUMBER

Your NHS number is a 10 digit number, like 485 777 3456.

You can find your NHS number in any documents or letters sent to you by the NHS, such as your:

  • prescriptions
  • test results
  • hospital referral letters
  • appointment letters

You do not need to know your NHS number to use NHS services, but it can be useful to have it. 

You can find your NHS number online by using the Find your NHS number service or by using NHS App.

PHYSICAL ACCESSIBILITY

Wheelchair access

Our practices are accessible to patients using a wheelchair. We also have parking spaces outside our practices which are reserved for patients displaying a disabled sticker.

Hearing difficulties

For patients with hearing problems, we have a hearing loop in the reception and main waiting room. There is also a portable hearing loop available from reception. Please remember to adjust your hearing aid accordingly.

Other needs

For any other needs, you or your carer should request help at our main reception desks where we will try to assist you.
